Healthcare services in general practice and the community are being transformed to make it easier for our patients to access a wider range of support.

As part of the NHS Long Term Plan, more healthcare professionals are joining networks of practices to provide greater choice and more seamless, integrated care. These specialists include Clinical Pharmacists, Physiotherapists, Paramedics, Health and Wellbeing Coaches, and Social Prescribing Link Workers, enabling you to access more tailored services closer to home.

The Additional Roles Reimbursement Scheme (ARRS) is expanding the range of support available. By funding additional clinical and non-clinical staff, ARRS helps PCNs offer faster, more specialised care. This team strengthens the surgery’s ability to address a wider variety of health concerns while freeing up GP appointments for those who need them most.

The ARRS team are a valuable asset, enhancing our capacity to deliver high-quality, focused care for the benefit of all our patients.

PCN Leadership Team

PCN Leadership Team

The PCN leadership and management team is primarily responsible for the safe and effective delivery of core network activities, including strategic planning, operations, finance, digital infrastructure, relationship management, HR, and contractual obligations like the DES and is led by a Clinical Director, who in our case is a GP. The team works across multiple practices, navigating varying cultures, patient needs, and structures. Our team is made up of the following:

  • Dr Nick Yeats – Clinical Director

Dr Yeats oversees the strategic and operational delivery of the PCN’s core activities. He provides clinical leadership for strategic planning, collaborates with member practices to enhance patient care and service quality, serving as the network’s lead representative and providing clinical oversight.

  • Ryan Smith – Strategic Manager

Ryan oversees key operational, financial, and leadership functions within the network and is responsible for delivering and monitoring commissioned projects and services, ensuring strong financial management, regulatory compliance, and effective resourcing. He also supports strategic transformation and change management across member practices.

  • Emily Marshall – PCN Operations Manager & Digital Transformation Lead

Emily leads the operational delivery of key services across the Kenilworth and Warwick PCN, including Enhanced Access, ARRS workforce development, and digital transformation programmes. Emily works closely with clinician leads and practices to improve patient outcomes, reduce inequalities, and drive service transformation, focusing on workforce planning, regulatory compliance, and leveraging digital solutions to enhance patient care.

  • Fay Brown – Personal Assistant to PCN Leadership Team

Fay supports Nick, Emily and Ryan by managing their schedules, organising meetings, and handling communications. Fay takes meeting minutes, maintains records, and coordinates with healthcare professionals, external partners, and other stakeholders to ensure smooth operations, allowing the leadership team to focus on patient care and strategic initiatives within the PCN.

Care Co-ordinators

Care Coordinators working in practice reception teams are specially trained to know about the care and services available to you at your surgery and in your area. While the name of this role may differ in your practice, they’ll listen and talk to you in confidence about your health problem, to understand your needs so they can book you an appointment with the right healthcare professional or service.

They can help you:

  • Get seen as soon as possible
  • Know whether self-referral is available for certain services at your practice or in your area
  • Make appointments for new kinds of care or new services you may not be aware of
  • Access the appropriate healthcare professional

Clinical Pharmacists

Clinical Pharmacists in your practice are highly trained professionals specialising in the safe and effective use of medications. They work closely with your healthcare team to review and manage your prescriptions, providing expert advice on medication-related issues. Whether it’s adjusting dosages, identifying potential side effects, or offering guidance on how to take your medications correctly, Clinical Pharmacists are here to ensure your treatment plan is optimised for your health needs. They can also assist with managing long-term conditions and provide education on managing your health through medication.

First Contact Physiotherapists

First Contact Physiotherapists in general practice are experts in musculoskeletal conditions. They are able to assess, diagnose and treat a range of complex muscle and joint conditions – preventing the need for referrals to hospital. They can arrange swift access to further treatment, investigations and specialists when needed.

They can help with:

  • Diagnosing and treating muscular and joint conditions
  • Advising on how to manage your condition
  • Referrals on to specialist services

Paramedics

Paramedics work in a variety of roles within a general practice. Their background in pre-hospital care means that they are used to working with people with a variety of health conditions from coughs and minor injuries to more serious conditions such as asthma and heart attacks.

They work alongside GPs and help manage routine or urgent appointments, telephone triage (assessment of urgency of illness or injury) and home visits.

They can help with:

  • Diagnosing and treating health conditions
  • Ordering tests and interpreting results
  • Supporting people with long-term conditions such as arthritis and diabetes

Social Prescribers

Social Prescribers help people to improve their health and wellbeing by connecting them to activities in the community. They also help people who are feeling lonely, overwhelmed, or in need of help to a range of local support – from community and activity groups, to work, debt or housing advice.

They can help with:

  • Getting people to focus on their own priorities and the things that affect their wellbeing
  • Supporting people to take more control of their health
  • Introducing people to groups and activities in their community

Frailty Nurses

Frailty Nurses in general practice specialise in the care of older individuals experiencing a decline in their health. Our nurses work in various settings, including community and in care homes, to assess, plan, and implement care for these individuals and focus on helping older people maintain their independence and quality of life, often working with multidisciplinary teams to provide comprehensive care. 

Pharmacy Technicians

Pharmacy Technicians are essential to the smooth running of pharmacies and work under the supervision of a pharmacist.

Their typical day could involve giving medicines to patients, either on prescription or over the counter and providing information on potential side-effects. Pharmacy Technicians will work with patients to reach the best possible care for them, advising them on their health, how to take their medicines and their lifestyle choice. This could be face-to-face or over the phone.
